Please answer the following questions for us so that we can get a good idea of your needs as well as the day your diaper deliveries will take place.

With this information, we can schedule you for an initial home visit, where we bring you the first week supply of diapers as well as go over a little more information.  The start-up fee/deposit of $20 is due at that time, as well as the first week's payment, so a total of $40 is due on the day we first meet.
